AWA New Member Intake Form

Congratulations, you are now a member of the AWA! Be sure to check your email for your receipt. The receipt will contain a link to register and access everything inside your member area. Complete the form below to set up your Member Landing Page. Once your information is submitted, your page will be live within 15 business days. Note: Your information will be displayed publicly online. Only share information you want displayed publicly.

What is the name of your business? This will be added to the top of your AWA Member Landing Page.
What is your business address? This will be how people can search for businesses close by on our website.
What is the best phone number a potential client can reach you at?
What email address do you want the form submissions to go to from the AWA website?

The “About My Business” is the perfect place to put information about your business. It’s where you can explain what you do, your history, and more about yourself and how you started your business. People should be able to read this page and learn more about you and get a deeper understanding of you and your skills.

This step is not required but is an added benefit to all AWA Members. To receive a blue checkmark on your AWA Member Landing Page, our AWA team must verify that you are licensed. Upload your license here. Your first and last name must match the name on your license and your license must be up to date. Once we verify your information, your “Licensed” blue check will be shown on your page.

This step is not required but is also an added benefit to all AWA Members. You have the option to get your pink “Verified” badge on your Member Landing Page. All our AWA team has to do is verify your identity. This also builds trust for customers who may want to hire you. All you have to do is upload Proof of Identity, such as your driver's license.
